Mechanism of Operation of High Risk Patient Data Trace and Recall System
in the Hospital

Background:

Establishing a tracking and recalling system for high risk patients' early detection of their clinical situation. the doctors can get abnormal diagnostic reports immediately, it also can recall patients for urgent cares is critically important for today’s medical care institutions.

Materials:

Using the informatics of the High Risk Patient Data Trace System, for preventing the medical adverse events and medical errors.

Results: 

Among the 10608 cases tracked by the High Risk Patient Data Trace System, 99 cases (OPD 86, ER2, and hospitalization 11) required to be recalled for the further attention. Of the 90% contacted and informed with suspicious or abnormal results of diagnosis, 9% was contacted by attending physicians and 91% by the Patient Referral Center.  One of the cases that is worth mentioning concerned a 76 year old woman who had an MRI done, due to a fall, in the Neurological Internal Medicine outpatient clinic.  When the radiologist found out the patient had a progressive SDH in her MR image, a neurologist was called and the patient’s family contacted to immediately bring the patient back to the hospital.  At the same time, we contacted a neurosurgeon to prepare for an emergent surgery for the patient.  As a result of this team work, the life of the patient was saved.

Conclusions:

An effective tracing and management system that can provide prompt, positive, and quality cares for patients can not be neglected.  The implement of an effective mechanism followed by adequate executions can promote early discoveries of sentinel events, thereby preventing medical adverse events and medical errors.
