Clinical Nurse Specialists' Practice Patterns: Activities, Outcomes and Barriers

Through their diverse roles that encompass practice, research and education, quite possibly, the Clinical Nurse Specialist is the one advanced practice nurse who has the greatest potential to impact the health of patients and families. Through clinical decision making, local policy work, and mentoring nurses throughout the continuum of care they can impact health care outcomes. However, professional literature continues to document gaps between the ideal and real world experiences of CNSs as they attempt to meet patient, staff, and organizational needs, as well as, fulfill the five traditional role components (expert clinical practice, education, research, consultation and clinical leadership). Our own CNSs were verbalizing role conflicts and a good number had left our organization over the years. So, our organization, a large California HMO, was searching for ways to best deploy these advanced practice nurses. In fact, we wanted to know how CNSs throughout California (CA) were operationalizing their roles and impacting health care outcomes. The timing was perfect -- the CA BRN was ready to embark on an assessment of CNS demographics and practice. So, our research team partnered with the CA BRN and conducted a multi-part study in 2002. This symposium presents the findings, conclusions, and suggested applications derived from that three-part survey study examining roles and activities, outcomes, and barriers to practice among 945 California BRN certified Clinical Nurse Specialists.
