Addressing One Southwest Georgia Community's 'Silent Anguish'...Integrating Strategic Planning, Research, and Education to Impact EOL Care practices.

Learning Objective #1: Identify significant socio-demographic and health system contextual factors and the steps involved in effective strategic planning for EOL community improvement initiatives
Learning Objective #2: Appreciate the significance of evidence based population research with African American and Caucasian surviving families of decedents for establishing the education and practice strategies of a diverse volunteer stakeholder group engaged in efforts to improve EOL care
The purpose of this symposium presentation is to describe one community's efforts to bring to life the vision, "Loving care for all at the end of life." It will demonstrate both the complexity and potential benefits of integrating strategic planning, research, and education to impact end-of-life health system resources and practices. Approaches take into account multiple interacting cultural influences including, but not limited to racial/ethnic, regional, and professional subcultures, and are grounded on the basic assumption that defining quality in EOL care practices is the perogative of patients, families, and other survivors. Results will demonstrate that establishing community ownership for health system change is a critical, but long-term process, requiring focus, persistence, monitoring and evaluation, and periodic revision of community wide priorities.
