Heart to Heart with Mothers and Daughters: Focus on Breast Health is a model program designed to foster effective communication between mothers and daughters regarding breast health through the lifespan under the guidance of school nurses. The session encourages girls to view their mothers as supportive persons during the tumultuous teen years. This special "just for us" time is a celebration of the bond between mothers and daughters and their shared experience of maturing. The goal is to encourage daughters to see their mothers as resources for questions about breast health. Effective communication is key during the transition from pediatric to adult health services. The program includes icebreakers, communication exercises, information about breast health and self care, and how to maintain effective communication with a health care provider. The target audience is girls ages 13-18 years old with their mothers/significant female adults in high poverty schools. Some sessions are tailored to the needs of the Hispanic community and include a translator. Each mother/daughter pair attends one session. A toolkit of materials was developed for school nurses to use in extending the model program to other schools.
