It's necessary to have long term self-care of the drainage for cases of percutaneous transhepatic cholangiography with drainage (PTCD). The purpose of this strategy is to set up a uniformly nursing care standard for cases of PTCD in order to promote the quality of nursing care and reach nursing goal of best self-care. Analysis of the current problems are low skin integrity, wound care uncomfortableness and pain, High drainage tubing slippage rate, Low quality of nursing care. The project was set up a nursing care standard , utilization of Y type IV Tegaderm dressing to substitute tradition gauze, provide a guide for wound care and evaluation. In conclusion, the skin integrity percentage of wound increased from 20% to 88%, comfort level of patients upgraded from 4.79 to 9.56, average pain degree of patients descended from 6.10 to 2.91, dislodge rate of drainage tube reduced from 9% to 0%, and integrity of nursing care elevated from 81.5% to 100%.We hope this project may be useful for nursing colleagues as reference in clinical application.
