Gynecological Care of Elderly Women in Assisted Living Situations

Historically, gynecological care of elderly women has been ignored by the health care establishment. This has been particularly true of women in nursing homes or other assisted living arrangements for the elderly or disabled. Women in these situations are often viewed as asexual by caretakers and health care professionals alike. Legitimate questions and concerns about sexual or gynecological issues usually are not addressed.

Nurses can provide leadership in the provision of gynecological care and education to this vulnerable population. Older women are at increased risk for a number of gynecological problems. These include reproductive malignancies, prolapse of the pelvic organs, and atrophic vagical changes leading to irritations and infections. Often these problems go undiagnosed and untreated, leading to unnecessary suffering and/or premature death. In addition, many women in assisted living situations suffer from social isolation, lack of privacy, and caretaker abuse.

This presentation will describe the gynecological needs of elderly women and demonstrate how nurses are beginning to address these needs. Suggestions for future research, education, and practice strategies will also be presented.
