Community-Based Health Care for Asian Elders: An Academic/Community Partnership Model

Purpose and need:The need for developing practitioners who can assess the needs of diverse populations and implement health care and education is critical.Providing experiences and building relationships with community centers expands the community health nursing experience for students and provides a mechanism for transitioning to a population/community based practice.

Methodology: This presentation focuses on the partnership created between community neighborhood centers and community health nursing clinicals using an academic nurse managed center model to provide student educational experiences and health care services to the elderly Asian population of an urban area of Northern California. Topics to be discussed include: 1) effectiveness of the setting as an arena for nursing students to synthesize theory and practice; 2) strategies for providing health care to a target population of low income Asian elders; and 3)developing and maintaing linkages with the community center as an educational/service site.

Conclusions: Changes in the health care delivery system have required expansion of nursing education into the community. This expansion has provided opportunities for collaboration with community agencies and implementation of innovative nursing curricula.
