This study is to describe the process of developing an instrument designed to study the nurse’s intention to advocate for patients. Although advocacy is an essential component of the registered nurse’s professional role and has been discussed frequently in nursing literature, experts provide no consistent definition of advocacy. Also, there are limited empirical studies about nurses’ advocacy practice, and little is known about the factors influencing nurses’ advocacy role. The aims of this study are to: (1) develop an instrument to measure the nurse’s intention to advocate and the factors influencing his or her intention to advocate; and (2) establish the psychometric properties of the instrument. The process of this instrument development consists of three stages. The first stage is the refinement and operationalization of the concept of advocacy using Walker and Avant’s concept analysis method. The attributes of advocacy are identified. The second stage is the theory development. A model combining two models serves as the theoretical framework for conceptualizing the relationships between the nurse’s intention to advocate and the factors influencing his or her intention to advocate. One is the Advocating Model constructed by the author to explain nurses’ advocating behaviors based on the results of the concept analysis of advocacy. The other is Ajzen’s Theory of Planned Behavior (TPB). The third stage is the instrument development and psychometric testing. The instrument measuring intention and the factors influencing the intention will be developed based on the results of concept analysis of advocacy and the advocacy literature review. The instrument will be piloted using about 200 clinical practicing nurses. Results of psychometric testing to determine the validity and reliability of the instrument will be reported. This developed instrument can be used to test the combined model. Then the intervention to facilitate nurses’ advocacy role can be developed.
