This course will guide the audience in analyzing two cases taken from the ED of a university teaching hospital where the presenter discovered serious medical disorders causing psychiatric behavior during the medical screening exam (MSE). Advanced practice nurses that work in ED’s need to be aware of potential pitfalls at triage that result in the inappropriate assignment of patients to psychiatry. Understanding the importance of medical screening examinations in preventing increased patient morbidity and mortality as well as its role in protecting NP’s from liability claims, is a topic not contained in most graduate curricula. The group will also discuss the ability (or inability) of many healthcare providers to recognize common medical illness in patients with co-existing psychiatric diagnoses. In addition, the presenter will outline factors that confound recognition of overt medical disorders in patients with previously diagnosed psychiatric illness and how advanced practice nurses can avoid these pitfalls. Citations from the psychiatric and emergency medicine literature will be used to support material presented.
