Learning Objective #1: Describe the components of Brazilian public health system and nursing care delivery |
Learning Objective #2: Understand how a collaborative partnership was developed between community health nurses and faculty of a nursing school to improve clinical practices related to wound care |
In Brazil, the public health system consists of institutions affiliated with the federal, state and local governments, ranging from primary health care to secondary health centers and large hospitals, which are arranged hierarchically by level of complexity. Together with 24 other cities, Ribeirão Preto, a city with an urban population of 534,944 inhabitants, constitutes the XVIII Regional Health DIR of the São Paulo State Health Secretary. The municipal health network includes 35 Basic Health Units (BHU), which are responsible for primary and secondary health care delivery to the population covered by its activity area. Furthermore, 18 Family Health Teams deliver preventive and curative home care. At the end of the 1990's, there was a significant increase in the care demands of patients with chronic wounds, both in homecare nursing and in the wound care clinics at the city's BHU, mainly involving patients discharged from hospital with relatively complex home care needs. Nurses of health units observed the need to form a multiprofessional team to improve the quality of care, to update workers knowledge and standardize practices. In order to do that a group of nurses tried to start a close collaborative partnerships among themselves and with a faculty member at the University of São Paulo at Ribeirão Preto College of Nursing who researches on chronic wound area setting up a commission to organize courses and establish protocols with a view to evidence-based practice. Since 2000, the commission has organized courses for health professionals in different cities. To disseminate the protocols to the health professionals, a manual has been elaborated and distributed across the institutions, while the eletronic version as a PDF file has been made available on the Health Secretary website. A study is currently in course to examine the nurses' adherence to the care practice recommendations.
