Learning from mistakes is important to maintaining and improving the quality of care and patient safety. Reporting and investigation of incidents is by its very nature a hasty process. The challenge for Aga Khan Health Service, Pakistan is to turn this negative phenomenon into a positive opportunity for growth through the culture of learning by mistakes. The presentation will cover the relationship of consequential mistakes and non consequential mistakes, discusses different studies conducted and many barriers to reporting. Also discuss the system for collecting incident data this will enable them to analyze the type, frequency and severity of the incidents and to use this information to improve system and clinical care. On other side, we can turn the learning into a practical learning tool approach by reporting and investigating all types of incidents.
