Self-Management Issues for Diabetes Care
Learning Objective #1: To know the maijor issues of diabetes self-management among inpatients and outpatients.
Learning Objective #2: To learn some creative health education program for diabetes prevention.
Prevalence of diabetes complication was found high in Hualien, which located in eastern Taiwan. Case management was applied to improve quality of care in diabetes individuals who hospitalized because of their complications. There was significant differences (p <.05) in diabetes knowledge (7.4±1.2), self-management activities (51.2±8.6), inpatient satisfaction (4.5±0.5), and A1C (7.6±1.8). We recommended that case management model could be applied broadly among inpatients with diabetes. However, lack of self monitoring blood glucose (SMBG) was found no effective outcome even in the intervention group. We investigate the related factors that influenced SMBG behavior and found that social support accounted for 49.7% and health belief accounted for 11.4%. Family is usually a common resource of social support. Diabetes patients also have family history of diabetes. Pre-diabetes is not unusual among the family. We conducted a quasi- experimental study to examine the effects of a convenient and creative diabetes-preventing health education on people with pre-diabetes. The Results showed significant improvement in knowledge of diabetes prevention and exercise and diet prevention behavior in experimental group. The case management and multi-approach health education will be a good program for diabetes control and prevention. Some other intervention should be created and tested the effects on other self-care issues of diabetes.
