The Impact of Diabetic Related Retinopathy and Neuropathy on Patients' Falls

Purpose: In view of the increasing prevalence of diabetes mellitus worldwide, diabetes mellitus (DM) induce retinopathy and neuropathy are known to influence patients’ quality of life and safety, such as falling. However, it is lack of information known in this field. Thus, the purposes of the study were to examine (1) the severity of DM related neuropathy, and (2) the influence of DM induced neuropathy and retinopathy on patients’ fall.
Methods: A cross-sectional study was conducted in a medical center in Taipei. A total of 150 diabetic retinopathy patients were recruited. Visual acuity, standing balance and Michigan Neuropathy Screening Instrument(MNSI)were assessed. Falls were defined by retrospective self-report over 6 months period.
Results: The findings showed that (1) The most common diabetic neuropathy sensory symptom is pain and numbness. The most common abnormal neuropathy sign was light touch sense.(2) While using patients binocular visual acuity difference<0.6 and MNSI clinical examination score<4 as baseline, patients had (a) MNSI clinical examination score>4 and binocular visual acuity difference > 0.6, the fall risk significantly increased to be almost three times; (b) MNSI clinical examination score <4 and binocular visual acuity difference > 0.6, as well as the MNSI score>4 and binocular visual acuity difference <0.6 both had significantly higher risk of fall.
Conclusion: The study provided information about the risk of falling among DM patients with retinopathy and neuropathy. Overall we hope that the finding will improve the quality of life and reduce falls.
