Purpose:

Pain is a kind of many aspects, complicated and subjective experience and description. The Joint Commission on Accreditation of Heathcare Organizations has listed pain as one of vital signs in 2001. Correct evaluation in pain is one of the most important factors in pain care quality. The purpose of this study is to construct a standard in evaluation, recording and pain care , and also to provide correct and adequate patient care in time.

Methods:

This study in quasi-experimental design enrolled patients, who was diagnosed myocardial infarction and unstable angina, from Nov.1, 2008 to Nov.30, 2008 at heart failure center of one academic hospital  in Taiwan. The pain care standard of myocardial infarction as intervention was constructed after literature review, expert opinion and team discussion, and was applied for one month in December, 2008. We investigated the confidence of clinician and the patient’s satisfaction in pain care by anonymous questionnaires (100 scoring scale) before and after the intervention.

Results:

The confidence of doctor and nursing staff is 82 and 78 in score respectively before the intervention. The patient’s satisfaction in pain care is 78.2 in score before the intervention. After the standard pain care of myocardial infarction, the confidence of doctor and nursing staff elevate to 94 and 92 in score respectively. The most increase in score is 12.8 among junior clinicians less than 2 years experience. The patient’s satisfaction in pain care reached 89.5 in score after the standard pain care.

Conclusion: Acute myocardial infarction often present as chest pain. The ability of pain evaluation is important to relieve patient’s pain in time. The pain care standard construction is helpful to aid junior clinician in pain care.
