International Consortium Outcomes from Using a Professional Practice Framework to Guide Transformation

Purpose: This presentation will focus on the impact of shifting from a singular project driven change approach  to one that also includes a professional practice framework driven change mindset and will show the impact in sustaining positive change outcomes.  The insights will reflect the lessons learned in an International Consortium of over 280 rural, community and university settings being guided by the professional practice framework.  

Methods: An overview of the International Consortium members and the specific actions  taken to support transformation at the point of care using a practice framework will be provided.  Specific actions steps taken by the clinical and academic settings will be correlated to the components of the framework including interdisciplinary, integrated, evidence-based practice and education processes.

Results: The difference between the clinical, financial, safety and quality outcomes for those using a framework will be compared to sites not using the profesional practice framework. These will be correlated to National Quality Indicators, credentialing, JCAHO and Magnet criteria.    

Conclusion: The ability to create sustainable outcomes is markedly influenced by transformation that is driven by a professional practice framework that begins within the educational process and is supported in the clinical practice setting.
