Learning Objective 2: reduce the incidence of nosocomial infections of urinary tract
Nosocomial urinary tract infections will not only increase patient morbidity and mortality, but also to extend a patient's hospital stay, higher medical costs (Holroyd-Leduc & Sen & Bertenthal, 2007), the purpose of this project in order to reduce nosocomial urinary tract infection rate. 2009 January to March of nosocomial urinary tract infection rate averaged 3.46 ‰, the investigation in 35 patients, average age of 70.7 years, those indwelling catheters who have received 100% of the average number of hospital days 37.4 days, an average of 2.86 times of re-placement of catheter, and retention of 20.1 days. Analyze the reasons are: the completeness of foley care less than 77.5%, patients and their families lack of recognition of catheter care, only 60.4%, lack of equipment and a shortage of materials (wheelchairs ,toilets linked, plastic pipe clip and hand-washing equipment), lack of catheter patient care processes and health education information. The development of improved responses: 1. Making bedside catheter care health education card: 32 words within the text to support simple and easy to understand picture of the text description. 2. The establishment of catheter care procedures. 3. Organize training courses to prevent urinary tract infections and group health education. 4. An additional toilet in the bathroom next to the wheelchair urine bag hooks and hand-washing facilities. Through the project’s implementation ,nurses caring for the completeness of catheter raised to 100%,patients and their families’ awareness of the catheter care raised to 98.4%,nosocomial urinary tract infection rate decreased to 2.35‰.Showing nurses based on catheter care procedures, health education card provides bedside care guidelines, is able to avoid unnecessary indwelling urinary retention, can reduce the incidence of urinary tract nosocomial infections, thus contributing to the quality of nursing care.
