Evidence-Based Nursing and Diagnostic Accuracy
Learning Objective #1: Apply an evidence-based model (EBM) to improve the accuracy of nurses' interpretations of patient data
Learning Objective #2: Use an EBM and case studies to identify the best available evidence for exemplar diagnosis
In the current health care environment, evidence-based practice (EBP) is expected of all disciplines. Nurses who are involved in EBP generally focus on the efficacy of interventions, but, with diagnostic accuracy as the basis for interventions, interpretations of patient data should also be evidence-based. Sackett et al.'s (2000) five steps to identify the best available evidence are also useful as an evidence-based model (EBM) for nurses to identify the best available evidence for diagnostic accuracy in specific patient situations. In the past, nurses assumed that their interpretations of patient data were highly accurate as shown by the lack of discussion in the literature about concerns for diagnostic accuracy. Many studies in the past three decades indicate, however, that accuracy of nurses' diagnoses varies widely, from low to high. The authors argue that EBP will not be achieved unless nurses' interpretations of patient data are examined for the best available evidence. Diagnostic accuracy in nursing is particularly challenging because of the holistic nature of nursing phenomena, e.g., body image disturbance, changes in sleep patterns. The presentations in this symposium will 1) synthesize research findings on diagnostic accuracy; 2) apply Sackett et al.'s five steps of examining the best evidence to diagnostic accuracy in nursing; 3)apply the EBM for diagnostic accuracy using the exemplar diagnosis of Anxiety; and, 4) use a case study approach with other diagnoses to teach EBP for diagnostic accuracy.
