Learning Objective #1: Identify that physical restraint is not the only strategy to prevent unplanned extubation |
Learning Objective #2: Provide a safety environment, include physical and psychological aspects, to prevent unplanned extubation |
The purpose of this study was to find the related factors of the patient who unplanned extubation in ICU, to confirm the couse of the incidence of unplannedextubation, and to improve the safety of patient care in ICU. This was a retrospective studyfrom Jan. 2002 to Dec. 2003, a convenience sample of 139 patients of unplanned extubation, 79 in 2002 and 60 in 2003. The study was at a teaching hospital. The tool of this study was a report of unplanned extubation and the patient's chart, writed by doctor, nurse and respiratory therapist. We used SPSS for window 10.0 to analyse the data. The results showed that there was statistic differencein 2002 and 2003(P=0.021), and no statistic difference in medical and surgical ICU(P>0.05). Physical restriant use and sedation of narcotic were correlated with unplanned extubation. (P<0.05).Findings provide a important concept that physical restriant is not the only strategy to prevent unplanned extubation. We should understand the patient's uncomfortable and stress in ICU, and administer the sedative of narcotic properly. Then the safety of patient care could promote continuously.
