The Demise of "Sacred Cows" in Nursing Practice: A Magnet Hospital Uses the Evidence

The concept of evidence based practice was introduced within our 720 bed tertiary teaching hospital in the mid-1990's. An evidence-based approach was utilized by our trauma division chief as the optimum method for an interdiscplinary team to develop trauma clinical practice guidelines. At that time, nurses involved in this effort recognized the need to incorporate an evidence-based model within nursing practice. In 1998, three advanced practice nurses attended the Agency for Health Care Policy and Research (AHCPR) sponsored conference, "Translating Evidence Into Practice." These nurses then shared the concepts with colleagues and developed a formal evidence-based practice model to be utilized within our Network. The first true application of utilizing the newly defined model occurred in 1999. In response to the Institute of Medicine report, “To Err is Human,” we embarked upon a multidisciplinary, systems approach to reducing medical errors. The Primum Non Nocere (PNN) initiative consisted of 15 focused, practical projects to improve care through process redesign in order to decrease unnecessary procedures and reduce preventable adverse events. The “First Do No Harm” projects went further than traditional performance improvement projects in their quest, and indeed requirement, to use an evidence-based research methodology. Nurse and physician co-champions led each project team and reported regularly to senior management and the Board of Trustees, as well as to all hospital employees through a variety of communication strategies. As such, the term “evidence-based practice” soon became a well-known phrase within our Network. Indeed, our culture was transformed. Use of the evidence permeates everything we do. This symposium will provide an overview of our evidence-based practice model, examples of how we implemented the model and, focus on four individual projects in which the evidence unequivocably drove nursing practice changes in our Magnet designated hospital.
