A Model for Implementing Evidence-Based Practice in Vulnerable Populations in Acute and Community Settings

Learning Objective #1: Describe how an EBP model can be implemented, including strategies to overcome identified barriers to model implementation, the role of EBP in caring for vulnerable populations, and the cost of an EBP mentor for improving patient care
Learning Objective #2: Discuss the implications of implementing an EBP model for nursing and patient outcomes in an acute care and community care setting
Children and older adults are vulnerable populations with specific healthcare needs. These populations present unique challenges to nurses who provide their care. This symposium is comprised of presentations that will assist the participants in critical problem solving around issues experienced in their own practice. The first session will be the presentation of a model for promoting evidence-based practice in acute and community care settings. As well, there will be a short overview of EBP principles and how the model is based on these principles. The second presentation will provide the participant with lessons learned through implementation of the model, including nurse-identified barriers and strategies to overcome the barriers. The third presentation will report findings from a pilot study in which an expert in EBP mentored community health nurses in integrating evidence into their daily practice. The fourth presentation will report the findings of a pilot project in which an expert in EBP mentored nurses caring for adolescents on an acute care unit. Both pilot study presentations will include lessons learned from the project and focus on outcome evaluation of EBP implementation. Upon completion of the symposium, participants will return their institutions and communities with nuts and bolts to begin to implement practice change based on evidence.
