Abstract for Chiron Leadership Program: Detoxification Protocol Educational Modules for Nursing in a Non-medical Free-standing Psychiatric Facility

Learning Objective #1: identify knowledge deficits related to alcohol, opiate and benzodiazpine withdrawal and the administration of as needed medications to control the physiological symptoms of withdrawal.
Learning Objective #2: correlate the significance of alterations in vital signs and physiological symptoms to administer as needed medications to decrease power struggles between nursing staff and patients.

  The purpose of the project is the development of Alcohol and Opiate protocol educational materials specific to the need for detoxification of adults in an inpatient dual diagnosis setting. A knowledge deficit among psychiatric nursing practitioners related to detoxification protocols in a non-medical free-standing psychiatric facility was identified. This problem is addressed through the development of educational curricula designed for psychiatric nursing and mental health care technicians.
The rationale is to increase the safety of patients during medical detoxification by improving nursing understanding of detoxification medications.  Improved knowledge base will decrease the power struggle between health care staff and patient population. The project expanded to include mental health care technicians in the training curriculum related to obtaining vital signs.

Methods for the development of the curricula include identifying knowledge deficiencies using a questionnaire for nursing staff.  Medication administration records for as needed medications were reviewed.  The correlation between physiological symptoms of withdrawal and the administration of as needed medications was examined. This material has been reviewed with the pharmacy staff and medical director and has led to recommendations for the alteration of existing protocols. Anticipated results include decreased power struggles between nursing staff and patients over the administration of as needed medications related to knowledge deficits.  Knowledge deficits identified include:  mechanism of action of medications; correlation between elevation in diastolic blood pressure, elevation in pulse and tremors during withdrawal and the need for medication administration to control withdrawal symptoms; identification of physiological symptoms of withdrawal. In process is the development and implementation of Ativan in a detoxification protocol with standing orders sets; the implementation of revised order sets for existing detoxification protocols; new training materials for nursing staff related to vital signs and the implementation of new documentation for recording vital signs.
