Utilizing Evidence Based Practice to Improve the Incidence of Falls in Hospitalized Pediatric Patients.
Falls in hospitalized adult patients have been studied continuously since the late 1980s but falls in the pediatric patient population have received little attention in the medical and nursing literature.
Purpose: To develop an appropriate tool to assess pediatric patients who are at risk for falls in the hospital and implement the tool in the practice setting.
Methods: A retrospective chart review of 100 falls in hospitalized children has been conducted and compared to 100 pediatric patients matched for age and year of admission who did not fall. Data was collected utilizing the Morse Scale and the Hendrich II Fall Risk model as well as possible risk factors not included on these adult tools. Results: The results of this retrospective chart review were formatted into a Pediatric falls risk assessment tool. This tool was built into a computerized pediatric falls risk assessment by a interdisciplinary team for use with nursing documentation. The nursing shared leadership structure, with the help of pediatric unit based councils, developed education and interventions as a result of the information learned from this study. There is now in place the capability to identify ongoing patient safety needs, concerning falls risk, with a goal of improving patient outcomes through a computer documentation system.
