A Qualitative Study Evaluating a Pre-Operative Education Program for Colorectal Surgical Patients Requiring Permanent Ostomies

The Centers for Disease Control and Prevention Fact Sheet lists colorectal cancer as the third most common cancer in men and women and the second leading cause of cancer-related death in the United States. In March 2001 a new approach to patient education was implemented. This program included pre-operative teaching. Topics included in the preoperative teaching are:  pain management; diet (early feeding); activity (early ambulation); use of the incentive spirometer; prevention of deep vein thrombosis (sequential compression devices); and colostomy care. There were 47 participants in 2004, but no formal evaluation of the program has been conducted. Without evaluating educational programs and asking patients what information they need, effective, patient-centered care cannot reach its threshold.  Learning what patients need to know, can assist knowledge on how and what to teach patients. There is no research asking patients who have had colorectal surgery what there perceptions of the education they received or what educational information did they need, but not receive. The purpose of this study is to determine the patient’s perceptions of the effectiveness of the pre-operative teaching in preparing the patient to recover and cope after surgery. The research design is a qualitative, phenomenological approach. The patient population will consist of a convenience sample 20 patients undergoing pre-operative teaching for colorectal surgery requiring a permanent colostomy. The interviews will be semi-structured with identified, scripted, open-ended questions contained in the Interview Protocol.  Demographics will be analyzed using descriptive statistics. Data analysis will be conducted using the Colaizzi data analysis method.  Themes will be integrated into an comprehensive description. Interpretation of the data will be verified between researchers to confirm contextual meaning.
