Development of a Cognitive Behavioural Approach Education Program for Pain Management for Patients with a Fractured Limb and Undergoing Surgery

This project  is part of the Ph.D study of “The effect of an education program on pain management and post –operative outcome of Chinese patients with traumatic fracture limb undergoing surgery. This paper presents the process of development of a cognitive behavioral approach education program for pain management for patients with fracture limb and undergoing surgery
Procedure:
A qualitative study using phenomenological approach was used to interview 26 patients who had traumatic fracture limb and undergoing surgery.  This qualitative study aimed to   understand Chinese patients’ pain experience, belief and barriers of the pain management during their hospitalization in the acute setting.   Interviews were taped recorded and lasted for 45 minutes. Each transcript of the informant’s oral description of pain experience was analyzing following Colaizzi (1978) phenomenological methodology.

Result:Patients’ pain belief was one of the major barriers to adequate pain management.  The common pain beliefs were: analgesics were harmful to human body due to its side effects; pain medication should be used as the last resort and they should bear their pain as much as they could.  In addition, participants had insufficient knowledge related to the benefits of good pain management on their rehabilitation and recovery. The results of this study suggested that there is a need to modify patient misconceptions about pain management and increase their knowledge and self-efficacy in pain management
Implication: Based on above findings, the education program is developed using cognitive behavioral approach which aims at increasing patients’ knowledge on pain management; modifying patients’ negative thought and increasing patients’ constructive coping behavior on pain management.  The education program will promote better pain control; improve the physical and psychological outcomes of the patients.
