Moving Models to Reality: Applying a Model of EBP to Clinical Practice Guideline Development in a Specialty Nursing Organization

Evidence-based clinical practice guidelines (CPG) are systematically developed guidelines or statements designed to assist the practitioner and/or patient in making appropriate healthcare decisions in a specific clinical circumstance. Use of these guidelines has been shown to improve health outcomes of individuals. Development of these guidelines requires a painstaking process of problem identification and validation; exploration and retrieval of literature; rigorous review, critique, and synthesis of the evidence; design of a practice change; and then implementation and evaluation of that recommended change. The American Society of PeriAnesthesia Nurses issued a strategic call to EBP in the specialty in March of 2004, followed by the development and pilot testing of an EBP model and evidence evaluation tool in 2005. This paper presents the application of this model to the development and dissemination of an evidence-based CPG for the prevention and /or management of postoperative and post-discharge nausea and vomiting. The unique successes and challenges encountered during each phase of guideline development will be presented. Specific emphasis will be placed on identifying and engaging a multi-disciplinary team of experts with varied educational, clinical, and research backgrounds in this consensus building process. Challenges associated with time management, funding, and team communication will also be addressed. Finally, the final product will be presented with a summary of related dissemination and translation efforts to date.
