International Consortium Brings Evidence-Based Practice to the Point of Care for Thousands of Clinicians

Learning Objective #2: Delineate the structure and process essential to implement evidence-based practice in order to achieve desired clinical outcomes
This symposium will highlight the work being done in an International Consortium to implement evidence-based practice at the point of care by using a Professional Practice Framework and intentionally designed evidence-based tools and resources (both paper and automated) to support clinicians. One coordinator from a Canadian site and one from the United States along with the Chairperson of the Consortium will represent over 230 clinical settings who are collectively working together to create the best places to give and receive care by implementing infrastructures and evidence-based tools to support the thought and workflow of professional nursing. The focus will be on the nature of the implementation work necessary to assure culture readiness for evidence-based practice within the context of a practice framework, the involvement of staff in scholarly work, partnership infrastructures, clarity on scope of practice and interdisciplinary integration at all levels of thought and workflow. The clinical outcomes of the collective work will show the impact of knowing the patient's story, having a valued plan of care, prevention of complications and an education process that meets desired outcomes. The outcomes will be related to Canadian and American professional standards, federal, governmental, credentialing as well as JCAHO, Magnet, NQF and IHI initiatives.
