In Japan, the early separation from service of nurse-midwives working in hospitals is currently an issue. However, the process of career development of nurse-midwives working in hospitals has not been clearly treated theoretically.
Therefore, the purpose of this study is to investigate the career development process of nurse-midwives who have been working in hospitals for more than 10 years, in comparison to midwives who have chosen to separate from service after several years to set up in private practice.
Method
Design of study---Qualitative inductive study
The participants in this qualitative study were 7 nurse-midwives working in hospitals and 6 nurse-midwives in private practice.
Semi-structured interviews were conducted with the Nurse-midwives.“What were the experiences in midwifery practice, and what were the impressions?” was uses as the guide for interviewing.Interviews were transcribed verbatim to allow descriptive and inductive content analysis.
Period of study---From October,2008 to March,2009.
Ethical regard---This study was carried out with approval of the Ethics Review Committee of the institution of which the investigator is a member.
Result
The nurse-midwives working in hospitals came to have [conviction to perform midwifery practice as a team], whereas the nurse-midwives in private practice wished to put importance on [conviction to perform midwifery practice by oneself], but left the hospital because of being caught in a dilemma that this is difficult in reality.
Discussion
Nurse-midwives working in hospitals for more than 10 years perceived midwifery practice as a member of an organization and were developing careers as team leaders.
On the other hand, nurse-midwives in private practice were changing course from the role as a member of an organization to the role of carrying out midwifery practice on their own responsibilities.
