The Effect of Discharge Planning Using Case Management Model on Patients with Stroke

Purpose: case management service model for stroke patients, which aims to investigate: 1. Self-care ability, knowledge of the effectiveness of stroke. 2. Hospital stay and medical cost effectiveness. 3. Satisfaction with discharge planning effectiveness.

Methods: Data collected from January 2009 to November, is designed for moderate ischemic stroke total of 100 patients in the control group and experimental groups of 50, was admitted to hospital within 24 hours to complete "personal attributes questionnaire", admission and discharge in accordance with "NIHSS" assessment of neurological function, and the Israeli-Palestinian Barthel Index on admission, discharge, discharge evaluation after 1,3 months of four times, the patient was discharged the same day, records of patient days, medical expenses, complete stroke knowledge questionnaire, discharge planning satisfaction survey.

Results: The results showed the experimental group, the average length of stay compared with the control group, the average length of stay; average cost of hospitalization in experimental group compared with the control group; admission and discharge of the NIHSS, Barthel index scores compared with both groups progress; stroke knowledge scores of experimental group (90.2) than the control group (67.1) progress, p <0.005 and up significantly, discharge planning satisfaction in the experimental group (93.2 ± 5.03) than the control group (71.1 ± 2.73), p <0.000 up to significant .
