Factors Affecting BODE Index in Patients with Chronic Obstructive Pulmonary Disease

Purpose

This study was done to identify the factors influencing BODE index in patients with chronic obstructive pulmonary disease (COPD) in South Korea and provide baseline data for development of nursing intervention for COPD.

Methods

The participants were 180 patients with COPD at 6 general hospitals in South Korea. The data were collected by self-reported questionnaire from March 1 to November 31 of 2010. To analyze the data, SPSS WIN 18.0 program using percentages, means, t-test, ANOVA, Pearson correlation coefficients, and multiple regression was utilized.   

Results

The mean score for BODE index was 3.11±2.359 on a 4 point scale. The BODE index was correlated with age (r=.147, p=.024), duration since diagnosed as COPD (r=.269, p<.001), level of fibrinogen (r=.178, p=.008), depression and anxiety (r=.128, p=.044), self-efficacy (r=-.353, p<.001), sleep disturbance (r=.175, p=.009), and correct use of inhaler (r=.131, p=.040). Stepwise multiple regression analysis presented that 17% of the variance for BODE index was explained by the factors included in this study. Significant influencing factors of BODE index were self-efficacy of COPD management (β=-.298), duration since diagnosed as COPD (β=.198), and level of fibrinogen (β=.137). Self-efficacy of COPD management was the most important predictor of clinical competence (β=-.298).

Conclusion

BODE index predicts the mortality risk of the patients with COPD. The findings of this study suggest that self-efficacy was the most important influencing factor of BODE index. Researchers and health professionals should develop nursing interventions to reduce the severity and mortality rate of the patients with COPD.
