Development and Analysis of Care Guideline for Early Removal Urethral Catheter

Learning Objective 2: The learner will be able to apply the guidelines in the care process of urethral catheter removal.

Purpose: Patient with urinary tract infection is the leading incidence in nosocomial infections. The infection remarkably affects patients to exasperate disease, extend hospital stay, raise medical cost, and increase patient mortality. Urinary tract infection is closely related to the duration of indwelling urinary catheter. This study was to establish care guideline for the removal of indwelling urinary catheter. Methods: Empirical nursing research methods with systematic literature review and expert focus group meeting were used. Research tool was a questionnaire of the care guidelines for the removal for urinary catheter. Total 497 subjects included nine experts and nurses in north central regional hospitals or medical centers, Taiwan. There 21 items in the care guideline to be used in establishing early removal urinary catheter. Results: There were 95.2% of experts and 91.5% of the experts nurses and register nurses to agree the care guideline of removal urethral catheter. From January to June 2010, patients were successfully removed urinary catheter with successful rate 79%. The urinary tract infection rate was significantly decreased (from 10.11 0/00 to 3.88 0/00, t=3.03,p=0.04). Nursing application: Clinical nurses could follow the care guideline for the removal of indwelling urinary catheter earlier to reduce the risk of urinary tract infection and improve the quality of clinical care. In conclusion, the nursing care for early removal urinary catheter plays an important role related to prevent urinary tract infection which could further improve the patient safe, the quality of patient care, and reduce the days of hospitalization.
