Introduction: The Integration into the family of a person who is dependent on self-care, requires a set of new skills, for which, not all the families are prepared.

Purpose: Understanding of the transition process experienced by families when one of its members start be dependent on self-care.

Methodology: Qualitative: multicases (Yin, 2003) using Grounded analyze.

Results: By analyzing in depth the experience of these families, such as the conditions of this complex transition, a dimension emerges: the "difficulties". When families are returning home from hospital emerge difficulties in different aspects, the "care" and "face the situation." The "difficulties" in the "care" refers to "the amount of tasks at home" the "intensity" to "deal with the instrumental aspects of care," for example change the diaper and "deal with the confusion." The difficulties "coping the situation," relate to aspects such as the "unpredictability about the future," "do it alone" and "first time at home" which constitutes factors that inhibits the transition of the family.

Conclusions: The understanding of the condition of the transition and its variability in time is a decisive factor to provide care for these families, because it points out trends of demands. Over time, the initial difficulties are being addressed, but emerging new, according to the demands of the new care needs of dependent, more focused with the instrumental aspects of care.
