C 11 Million Hearts®: A Platform for Promoting Population Health, Education and Partnerships in Academia

Description/Overview: Million Hearts® is a national initiative launched by the Department of Health and Human Services to prevent one million heart attacks and strokes by 2017 by focusing care on evidence-based, low cost deliverables or the ‘ABCS.’ . As a partner in this initiative, the Ohio State University College of Nursing has founded the National Interprofessional Education and Practice Consortium to Advance Million Hearts (NIEPCAMH). NIEPCAMH is an association of academic institutions, community groups, and healthcare organizations who are committed to the advancement of Million Hearts and promoting population health through prevention and screening. As part of NIEPCAMH, a free, online Million Hearts® interprofessional educational module was designed and is available. The module is a didactic and clinical teaching tool for health sciences’ students and over 100 organizations have accessed the module. The module is a 4 step process: the user watches a series of 5 lectures, the user goes into the community and completes 10 Million Hearts® screenings, the user enters de-identified data from the screenings into the educational module, and the user completes a post-test and is certified as a ‘Million Hearts Fellow.’ The module has over 3,800 users and via the module, over 29,000 people across the United States have had a cardiovascular Million Hearts® screening. Demographic and biometric data collected during the first 18 months of screenings will be detailed along with a population needs assessment. Prompted by results of the first 18 months of data, a quality improvement project to enhance the module’s content and improve rates of lifestyle modification counseling and referrals to health care providers by module users was performed and will be highlighted. The educational module and associated screenings have been incorporated into undergraduate and graduate nursing curricula and community outreach.
