Development of Elder Neglect Risk Screening Instrument for Homecare Nurses

Background:

As the elderly population continues to rise, care burden is inevitably increased, the elder mistreatment events continue to be exposed. The study pointed out that elderly people who are mistreated will have three times rate of death and chronic diseases, and increase medical expenses about $5.3 billion annually. The most common type of elder mistreatment is neglect about 50-70%. Due to the traditional family concept, the difficulty in collecting evidence and the low notification rate, therefore, the elder neglect is even more concerned. Although there is a general assessment tool for the elderly in Taiwan, it has not been able to assess the elder neglect. Therefore, this study will develop a screening instrument for the risk of elder neglect for homecare nurses.

Purpose:

To develop elder neglect risk screening instrument for homecare nurses, and to test and verify the reliability and validity of the instrument.

Methods:

Firstly, through literature review and data analysis, we will develop a 5-point scale instrument, invite 8-10 experts and scholars to conduct content validity index, and collect cases after the expert validity index (CVI) more than 0.8. In this cross-sectional study, selected 25 home care institutions in Tainan City from 547 home care institutions in Taiwan for convenient sampling, and all homecare nurses of the institution are recruited for cluster sampling, until 200 cases were collected. The data were analyzed with SPSS 22.0. Descriptive statistics were used to analyze the demographic characteristics of the elderly and homecare nurses. The Pearson's r correlation coefficient was used to verify the criterion validity of self-made instrument and the Elder Assessment Instrument (Elder Assessment Instrument, EAI). Exploratory factor analysis was used to verify constructive validity. The internal consistency and the inter-group correlation coefficient (Intraclass correlations) were used to verify the intra-rator consistency and the test-retest reliability of self-made instrument.

Results:

In pilot study, the CVI is 0.92. This study recruited 23 cases in pilot study, the cronbach alpha is 0.69, the intra-rator reliability is 0.718, the test-retest reliability is 0.918.

Conclusion:

This instrument is important for early detecting elder neglect. It should be verified carefully and modify again and again.
