Improving the Care for Patients with Chronic Kidney Disease: A Community Based Approach

The prevalence of chronic kidney disease (CKD) has imposed a major of public health problem in many countries in recent years. The purpose of this paper was to delineate a program of study on the process of a community approach for clients with CKD. At the first stage, the experience of patients with CKD and their health seeking behaviors was explored. A qualitative approach was applied with a purposive sample of six dialysis patients and seven of their families recruited in this study. An important phenomenon was noticed that most of the participants considered CKD could be cured. As a result, they would search all possibilities to restore their renal function, such as Chinese herbal medicine or alternative therapies. The strategies to manage the strong believe on “cure” would need to be included in the health education program. The second stage of this program of study, a longitudinal study was designed to evaluate a health education program for chronic kidney disease. A multidisciplinary pre-dialysis care program was developed. Sixty-six participants diagnosed with CKD were recruited. These participants joined the care program for health education. The results indicated that renal function did not show the significant difference for six month follow up. The satisfaction on their overall health status has shown a significant increased (p < .05) at the sixth month after joining the multidisciplinary program. Furthermore, exercise can be one of the effective interventions to promote the satisfaction on overall health status among participants. The third stage of the program, a multidisciplinary pre-dialysis care website was developed. The goal is to provide services to clients with CKD. In conclusion, the program should be implemented with a longitudinal approach. Using web technology may be one of the solutions for long term follow up program.
