The Establishment of School-Base Case Management Model for the Adolescents with Hypercholesterolemia

Background
Alarmingly Hypercholesterolemia has become a major health problem in adolescence in campus recently. Around 10% prevalence of adolescence with Hypercholesterolemia was reported in Taiwan. However, most researches conducted at the last decade were mainly on the study of the chronic disease students’ adaptation. There are very few studies concerning their health management.
Purpose
The major purpose of this study is to establish a school-base case management model for the student with Hypercholesterolemia, by which, the students would develop a healthy lifestyle, and their health status would be promoted. The model were not only benefit for enhancing the student’s health, but also for extending to the health management for other chronic diseases in school. 
 Methodology
This is a two-year research. The aim of the first year’s study is to focus on designing a case management model in school, including a health passport, individual health education, group health education, and resources reference. The second year, a quasi-experimental research methodology had been applied for evaluating the effect of the model.
Serum total cholesterol and student’s knowledge, attitude, self-efficacy and health behavior related to control cholesterol were investigated before the case management model, and post intervention. The program started from September, 2004 to June, 2005 .
    
Result
In total, 210 students with hypercholesterolemia from a Junior College in Taiwan were invited in this program, and were assigned into the experimental group ( n=91) and control group( n=119). The Results revealed: After case management model, the experimental group were significant difference in the score of knowledge and serum total cholesterol.  The response of degree of satisfaction was 89.6%.
Conclusion
The model should be beneficial for the participants in this program, it also be the base for a case management policy for the school health, to achieve the goal of health promoting school.
