Knowledge Integration: Embedding Evidence into an EMR

Learning Objective #1: Identify steps for moving evidence into an electronic health record.
Learning Objective #2: Describe barriers to successful knowledge integration.

Nursing evidence was translated into appropriate sections of the electronic health record through the use of electronic nursing documentation and on-line plans of care for nursing. The resulting electronic documentation and data collection has made it possible to use the evidence and make it appear as a seamless part of the documentation to the end user.

The steps to implementation include;

Identify clinical processes that surround the nursing evidence to be applied.            Perform gap analysis between existing content and nursing evidence            Determine design elements and impact on existing database            Complete design for orders, documentation, care planning and decision support.            Validate design with peers and researchers               Build the content in designated system            Test; does it work the way we expected it to work?            Import content into other domains where it will be operationalized            Implement the content following appropriate education of clinicians

Barriers:

Evidence must be discrete in order to make it executable within the system

Integration with existing database: new data designed incorrectly can break existing data elements.            Time and resources; it always is harder than you think.
